Hancock County HomeCare & Hospice nurse recognized

Angela Tobey

Hancock County HomeCare & Hospice (HCHC) has announced that Angela Tobey, RN, has successfully completed the Certificate for OASIS Specialist – Clinical exam. OASIS (Outcome and Assessment Information Set) is a data set required for use in home health agencies across the country to measure outcomes and processes, and to determine reimbursement for home health patients.

“We are very proud of Angela,” said Julie Bouchard, RN, COS-C, BSBA, director of quality assessment and performance improvement. “Having Angela receive the designation of COS-C means our other HCHC clinicians have direct access to a staff member that can both support their newly acquired skills and enhance improved OASIS accuracy scoring. This is a win for our patients as we use the tool to help create individualized plans of care that will provide the best outcomes from the services received by HCHC staff.”
